Important Notice About Documentation & Assessments

Lifecrafters provides outpatient mental health and substance use treatment. However, we do not provide disability evaluations, emotional support animal (ESA) letters, or documentation for workplace accommodations.

We also do not complete assessments or paperwork intended for use in legal or administrative proceedings, including but not limited to the Kentucky Court of Justice, AOC, or the Kentucky Department for Community Based Services (DCBS).

Clients may request access to their clinical records at any time, and records will be released in accordance with all applicable state and federal laws, including HIPAA and 42 CFR Part 2.
