Fees & Financial Policies
Transparent, ethical billing so you always know what to expect
1. Service Fees
Lifecrafters, LLC provides services in accordance with Kentucky Medicaid reimbursement guidelines and private-pay policies.
Individual Therapy (90837): $100.00 per session (private-pay)
Psychiatric Diagnostic Evaluation (90791): $103.63 per session
Psychoeducation Group (H2027, 2 hours): $369.52 per group session
IOP Group (H0015, 3 hours per day): $135.72 per day
2. Insurance & Medicaid Policy
Clients with active Kentucky Medicaid coverage will not be billed for covered services.
Private-pay clients are responsible for full payment at the time of service unless a payment plan is arranged in advance.
3. Payment Responsibility
If you use insurance, Lifecrafters will bill your carrier on your behalf.
Clients are responsible for copays, deductibles, and any portion not covered by insurance or Medicaid.
4. No-Show & Late Cancellation Policy
Appointments canceled with less than 24 hours’ notice or missed without notification may result in a $50 no-show fee.
This fee is not covered by insurance and applies only to private-pay clients.
Medicaid clients are exempt, in accordance with Kentucky Medicaid policy.
5. Refunds & Collections
Refunds for overpayments are issued within 30 days.
If a balance remains unpaid, Lifecrafters will make reasonable efforts to resolve it before referring to collections.
6. Sliding Scale Statement
Lifecrafters, LLC does not offer a sliding-fee scale.
Clients unable to pay will be provided referrals to community-based or low-cost providers.

8. Acknowledgment & Consent
By receiving services, clients acknowledge that they understand these fees and financial policies and may request a printed copy at any time.
