Through the client portal, you can:

  • Select an available appointment

  • Complete intake paperwork securely

  • Submit information before your session so we can prepare thoughtfully

  • Share your history, needs, and goals for treatment

As part of your intake, we will determine the most appropriate level of care based on your self-report, standardized screening tools, and a clinically informed assessment. This ensures that services align with your needs and with ASAM and AODE standards.

Important Notice About Documentation & Evaluations

Lifecrafters provides outpatient mental health and substance use treatment. However, we do not provide:

  • Disability evaluations or documentation

  • Emotional Support Animal (ESA) letters

  • Workplace accommodation documentation

  • Parenting or custody evaluations

  • Court-ordered or forensic assessments

However, with the client’s written permission (ROI), Lifecrafters can provide standard clinical documentation to local courts, such as:

  • Confirmation of the client’s level of care

  • Attendance verification

  • Treatment participation

  • Clinically appropriate progress updates

These communications are limited to treatment-related information only and must comply with HIPAA, 42 CFR Part 2, and all applicable state laws.

Clients may request access to their full clinical records at any time.
